District COVID InformationIf any student or student's household member has any one of the following symptoms, they should remain at home and contact the school nurse:
* Fever (over 100.4 degrees Fahrenheit) * Cough * Congestions * Headache (moderate to severe) * Shortness of Breath * Fatigue * Muscle or Body Aches * Sore Throat * Nausea/vomiting * Diarrhea |
Vision ExamsA vision exam is required for all students entering kindergarten. The vision exam is due by October 15th.

Physical Exam and Immunization Requirements
All students entering preschool, kindergarten or from out of state need to turn in a physical and up-to-date immunizations to avoid exclusion from school by the third Monday of September or within 30 days of entering school.

Food Allergy PlanIf your child has a life threatening allergy, please have your physician fill out an Allergy Action Plan for the school to follow.

Vision and Hearing ScreeningWill be completed for all Bethalto East students at some point during the school year. You will receive a notice by mail if your child does not pass the screening along with a form to be filled out by your child's health care provider. Please make sure to follow up on referrals promptly. Hearing and vision deficiencies can have a negative impact on your child's academic performance.

Medication at SchoolIf you child needs to take medication at school, a Medication Authorization Form must be completed by you and your child's physician. This form is required for over-the-counter and prescription medications. Check you handbook for a copy of this form.

Dental ExamsDental exams are required for students in Kindergarten. The exam must be turned in by May 15, 2021.
